I do not contract with any managed care or health insurance networks. I am considered an out of network provider by most plans.
Out of network services may be covered in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
What is my health insurance coverage and benefits?
What is my deductible and has it been met?
Payment for services provided are due at the time of your appointment. Cash, check, and major credit cards are accepted for payment. You will receive a receipt of service at the end of each visit which you may submit to your insurance plan for reimbursement. Alternatively, for your convenience, you may have my office manager submit the claim to your insurance company and have all reimbursements sent directly to your home.
The filing of additional reports, phone and letter correspondence with other entities such as employers, school systems etc. is not covered under health insurance. You will be charged for time spent in this capacity.

Services are by appointment only and your appointment is specifically reserved for you. If you need to cancel or reschedule an appointment, please call 24 hours in advance. Failure to attend a scheduled appointment or cancellation with less than 24 hours notice may result in your being billed the full appointment fee. Missed appointment fee is as follows: $500 for an initial consultation, $400 for a 60 minute session, and $300 dollars for a 45 minute session.
